Important Please keep this information with your personal records

  Monthly, I will cover a benefit that is offered by our employer. Unfortunately many of our membership and spouses are not aware of all the benefits that we have. We have not received our new SPD, but we can access it via the website at http://resources.hewitt.com/alcoa or we may contact them at 1-888-252-6212 (ALCOA123). Please email me or stop by if there is a topic you would like me to cover.

  Sadly, we have suffered the loss of our coworkers, who were very dear to us. Dealing with the loss of a loved one is overwhelming. Trying to navigate, and locate information should be the last thing on the mind’s of our families during their time of bereavement. The following information will help you, or a loved one with that process. Please share this information with your spouse, children or anyone that will need to handle your affairs.

·        Have all of your important documents together in one secure place.

·        Have copies of all life insurance polices and contact information for them.

  • Please designate a beneficiary for your 401k Savings Plan, as well as your Company Provided Life and Accidental Death Policies. If no Estate is established, the life insurance would be paid via line of succession.  (Spouse, children, mother/father, brother/sister, etc. - not necessarily in this order) 

·        Take advantage of our free Death/Living will preparation service.

  In the event of your death, your survivors will have a number of decisions to make regarding benefits. To begin the process, your survivor should contact your local human resources representative to report your death. The next call should be placed to 1-888-252-6212 (ALCOA123). You will be assigned a survivor advocate.

Once your death is reported, your eligible dependent(s) will receive a packet of materials showing your coverage and a password to use when contacting Info Line, and Your Benefits Resources (YBR) Web site. I urge you to share your password with your spouse, child or the person that you have designated to handle your affairs now so they can access your benefits right away.

Life and Accidental Death Insurance

  • Your survivor should contact MetLife (either through 1-888-ALCOA123 or directly) to receive information about your company-provided insurance benefits and any additional coverage that you have in place. MetLife can be contacted directly at 1-800-638-6420
  • Spouse and child life insurance will end unless your survivors choose to convert the coverage to individual policies. To do so, they must contact MetLife in writing within 31 days.
  • Under our plan we have employee life insurance of $44,000 for employee life.
  • We have $88,000 for accidental death
  • You can also, add or change your beneficiaries on the YBR web site
  • If you purchased the Optional Life Insurance from MetLife request a copy of the certificate of coverage if you have not received one. Your family can check your paycheck stubs under the after tax deductions to verify that you have this coverage if they are unable to locate the policy.
  • American Income life offered our membership a policy in case of Accidental Death. They can be contacted at 1-440-582-0040
  • The cost of the funeral expenses can be assigned to the funeral home.  MetLife will pay the funeral home only the "assigned amount" which would be the negotiated funeral cost.
  • In the event of a terminal illness you may receive benefits before death under the accelerated benefit option, upon death or as the result of an injury.

Health Care (Medical, Prescription Drug, Dental and Vision)

 Your surviving spouse and/or dependent children are eligible for six months of continued coverage under Alcoa's health care plans based on your location and the coverage you had in place. If you were eligible for retiree benefits at the time of your death, coverage for your survivors will continue under the retiree health care plan.

  • When Alcoa coverage ends, your spouse and/or dependent children may apply for continued coverage under the Consolidated Omnibus Budget Reconciliation Act (COBRA).
    • If eligible, your survivors will be sent information from Info Line.
    • Your survivors are responsible for paying the full cost of coverage plus a 2% administrative fee.

Flexible Spending Accounts (Health Care Fund and Dependent Care Fund) and Health Reimbursement Arrangement:

  • Health Care Fund: Claims for eligible expenses incurred before your death may be submitted, up to the annual amount you elected to deposit in your account. Expenses incurred after the date of your death are not eligible for reimbursement unless your spouse or estate elects to continue coverage through COBRA.
  • Dependent Care Fund:  Claims for eligible expenses incurred before your death may be submitted, up to the amount in your account at the time of your death. Expenses incurred after your death are not eligible for reimbursement. Your survivor should submit claim forms to:  Your Spending Account YSA P.O. Box 785040 Orlando, Florida 32878-5040

  • Health Reimbursement Arrangement: Claims forms may be submitted for eligible expenses incurred before and on the date of your death, up to the amount remaining in your HRA account. 

    If a dependent covered by your HRA is eligible for COBRA continuation coverage upon your death, your dependent may continue to use your HRA account to help pay for any premiums, up to the amount remaining in your account, for the remainder of the calendar year following your death.

    If no dependents are eligible for COBRA continuation coverage upon your death, any amount remaining in your HRA account will be forfeited.

Savings Plan

If you participated in the ALCOA SAVINGS PLAN your beneficiary will receive information about the plan, your vested benefit, and payout options. (Your beneficiary will be your spouse if you're married, unless you and your spouse both agreed in writing that it should be someone else.) You are automatically vested in the Savings Plan if you die while an active Alcoa employee.

 

Retirement Plan

 

 If you are vested in an Alcoa or participating subsidiary retirement plan, your surviving spouse may qualify for pension benefits. You become 100 % vested after 5 years of service under this plan. Your spouse’s pension benefit will be calculated according to the plan formula at normal retirement, at the earlier of the date you left the company or died. The benefit equals the survivor portion of the 50% joint survivor benefit you would have received at normal retirement age. *Please note that there are eligibility rules under our plan. Please review these under the pension plan rules on the YBR site or stop by and receive a copy from me. Under the pre-retirement death benefit for instance if you have 5 years of vesting service when you die, your spouse is eligible for benefits. He or she will receive pension payments if you die while actively employed or after leaving the company with deferred vested benefits. * You and your spouse must be married for one year before you die for your spouse to receive benefits.

 

Union / Company Resources If you die as an active employee UAW Local 1050 /Alcoa will take up a gate collection for your family. Your family will also receive a Bible from our union.
